Integrated Therapy Services for Children & Families
At Connected Care Therapy Group, we provide collaborative, client-centered care through a connected network of professionals including Occupational Therapists (OT), Speech-Language Pathologists (SLP), Physical Therapists (PT), and mental health providers.
Our approach is rooted in the belief that meaningful progress happens when care is intentional, coordinated when appropriate, and always centered around the needs of the individual and their family system.
We do not treat in isolation. We create connection across disciplines to support the whole child, whole adult, and whole family.
Our Interdisciplinary Care Model
At Connected Care Therapy Group, collaboration is available and used when it best supports clinical needs and outcomes.
Our interdisciplinary team may work together to:
Share insights across specialties (OT, SLP, PT, mental health)
Coordinate care planning for more complex or multifaceted needs
Support consistency across home, school, and therapy environments
Provide informed referrals within or outside of our network when needed
Strengthen understanding of the whole child or client across developmental domains
This level of collaboration helps reduce fragmented care and supports more connected, functional outcomes for families.
However, interdisciplinary collaboration is not required for every case. Many clients are best supported through focused, discipline-specific services delivered independently by the appropriate provider.
Regardless of the level of collaboration, every client receives individualized, evidence-based, and goal-driven care tailored to their unique needs.

Occupational Therapy
